Home and community habilitation is a program designed to support            individuals with intellectual disabilities by providing an hourly service to        integrate the individual into their          community and work toward            accomplishing a set goal.

Sample Goal Based Activities

-Shopping at the Mall -Attending a Sporting Event

-Bowling -Going out to Eat -Going to Public Library

-Exercising -Attending a Craft Class -Going to Local State Parks

-Going to a Block Party -Attending a Community Dance

An Example Scenario

If the individual’s goal is to learn to budget money, an activity would be to window shop for an item at the mall. If they have the funds, assist them in purchasing the item. If they do not, explain how to save until they have enough to buy it at a later date.
